The application process is two-fold: Employer Registration (mandatory for the establishment) and Employee Enrollment (done by the employer).
1. Process for the Employer (Establishment Registration)
ESI registration is mandatory for all non-seasonal factories/establishments employing 10 or more persons (or 20 or more in some specific states) whose employees earn a monthly wage of up to ₹21,000 (₹25,000 for persons with disability). The entire process is handled online.
Detailed Online Registration Steps
1. Sign Up on ESIC Portal: Visit the official ESIC Portal (esic.gov.in). Click on 'Employer Login' and then 'Sign Up' under the 'New Employer Registration' option.
2. Confirmation Email: Fill in the basic details (company name, Principal Employer name, region, email, mobile number). Upon submission, you will receive a confirmation email with login credentials (username and temporary password).
3. Fill Form 1: Log in to the ESIC portal using the credentials. Select the 'New Employer Registration' option and choose the 'Type of Unit'. This will lead you to the Employer Registration Form-1.
4. Enter Details: Accurately fill in all sections of Form 1:
- Unit Details: Name, Address, PAN, Commencement date, and Police Station jurisdiction.
- Employer Details: Name, Designation, and contact information of the Principal Employer.
- Factory/Establishment Details: Details regarding your business activity, existing registrations (e.g., Shops & Establishment Act), and date when the mandatory employee limit was reached.
- Employee Count: Total number of employees and the number of employees earning under the statutory wage limit (₹21,000).
6. Receive Registration Letter (C-11): Upon successful payment confirmation, the system automatically generates and sends the Registration Letter (Form C-11) to the registered email address. This letter contains the 17-digit ESI Registration Number, which serves as the valid proof of registration.
Documents Required (to be kept ready for online upload)
- Business Proof: Registration Certificate (under Shops & Establishment Act or Factories Act) or Incorporation Certificate/Partnership Deed.
- PAN: PAN Card of the Business Entity.
- Address Proof: Utility Bills (Electricity/Water/Gas, not older than 3 months) or Rent Agreement/Property Tax Receipt.
- Financial Details: Bank Statement or a Cancelled Cheque of the business.
- Statutory Documents: MOA and AOA (for Companies) or Trust Deed (for Trusts).
- List of Employees: Details (Name, Date of Joining, Wage, Father's Name, Address, and Date of Birth) of all employees.
The employee does not need to individually apply for ESI. Their enrollment is the responsibility of the registered employer.
Detailed Enrollment Steps
- Employer Collects Details: When a new employee joins the company, the employer collects essential details via the Employee Declaration Form.
- Employer Enrollment: The employer logs into their ESIC portal account and registers the new employee by clicking 'Register New IP (Insured Person)'. They enter the employee’s details, including Aadhaar, family members, and bank account information.
- IP Number Generation: Upon successful registration, the employee is allotted a unique 7-digit Insurance Number (IP Number).
- e-Pehchan Card: The employer is responsible for generating and distributing the e-Pehchan Card (or a temporary ID slip) to the employee. This card serves as the identity document for availing ESI medical benefits.
- Identity Proof: Aadhaar Card (mandatory) and PAN Card.
- Photographs: Two passport-size photographs of the employee and one of each dependent (spouse, children).
- Family Details: Details of family members for dependency coverage (Name, Relationship, Date of Birth).
- Bank Details: Bank Account Number and IFSC Code.
